Explore our different appointment options in the sections below. Functional medicine providers can help with many symptoms or concerns of health, along with health optimization. Examples of potential symptoms include hormonal concerns (menopause, perimenopause), autoimmune disorders (Hashimoto’s thyroiditis, arthritis, etc.), mold exposures, endocrine disorders, digestive issues (irritable bowel syndrome, Crohn’s, ulcerative colitis, heartburn, inflammatory bowel disease, constipation and diarrhea), metabolic concerns (insulin resistance, diabetes, weight changes, cardiovascular disease), as well as health optimization (prevent chronic disease and live your best life). At the Root also works with those who have chronic diseases and health conditions. This includes, but not limited to patients with cancer. We work with patients to strengthen immune systems, enhance detoxification, and provide support. Please note that we do not accept insurance, however every visit will generate a superbill, which you can submit to insurance for possible reimbursement. We would recommend you discuss this with your insurance carrier prior to a visit with us if you need further clarification. Additionally, we will accept flexible spending account (FSA) and healthcare spending account (HSA) money as a form of payment, if allowed by your insurance.

At the Root Functional Medicine prides itself in working closely with their patients to get to the bottom of things. One of the most important aspects in helping our patients is by listening and spending time with them. In most corporate office settings, healthcare providers are limited due to time constraints and pressures of seeing as many people as possible in a short amount of time. Because of this, important information from patients are often missed and care is not always optimal. We are proud to say that this is not an issue at At the Root Functional Medicine.

Pricing and appointment times are as follows for office visits:

New Patient Visit - 60 minutes of provider time. Ideally this visit will be in office. Appointment may be virtual if required by location. At this appointment, all patients will start with a 60 minute visit with their functional medicine provider. This visit will allow the provider to get to know the patient’s timeline and history. In functional medicine, we like to ask about the last time a patient “felt well.” Using this framework and determining the factors that contributed to less than optimal health is imperative to returning to wellness. During this visit, the providers could also recommend additional testing, make nutrition, lifestyle and possibly supplement recommendations. All of these recommendations are specific to that patient’s needs and concerns. Fee is $500 with Dr. Potrzebowski and $400 with nurse practitioner Alison Zolcak.

Follow Up Visit - 45 minutes of provider time. This visit can be in office or virtual. These visits are used to assess progress with recommended changes and also to review tests that were ordered at the initial visit. Additionally, we can use this time to continue building on healthy lifestyle changes. Time frame for follow up will be determined by the patient and provider. Fee is $375 with Dr. Potrzebowski and $300 with nurse practitioner Alison Zolcak.

Acute Visit Telehealth - 30 minutes of provider time. This visit is best for patients following up on well managed, chronic conditions or to assess straightforward medical concerns. Time frame for follow up will be determined by the patient and provider. Fee is $250.

Functional Medicine Office Visits

Why focus and restore balance to your gut health?

Your gut is the center of your health-it influences everything from digestion and immunity, to energy, mood and hormones, and overall wellbeing. It can effect every body system. Disease often begins in the gut, which makes it extremely important to have optimal gut health. This service with At the Root Functional Medicine dives deep into the health of your gut and examines the gut microbiome, which is the environment of the gut-the organisms that live within it (bacteria, microbes, etc.). These organisms help break down food, produce essential nutrients your body needs for health, support your immune system, and even communicate with your brain. Too much or too little of certain microbes can throw our bodies off balance and lead to illness or even disease. Unexplained bloating, fatigue, brain fog, headaches, skin issues, sleep troubles, mood changes, food intolerances, autoimmune diseases, and more can be associated with an imbalanced gut microbiome.

What sets this program apart from our typical office visits?

Each visit with the program is 1 hour long, whereas our typical follow up visits are 45 minutes long. There are 3 visits included in the program. In order to examine your gut microbiome, we use the GI-MAP test. This comprehensive stool test analyzes your gut microbiome by examining beneficial and pathogenic bacteria, yeast, viruses, fungi and parasites. It also examines certain markers for digestion, nutrient absorption, inflammation, and immune function. This test often explains the “why” behind certain health concerns. We use this test along with your specific concerns and comprehensive health history to determine a plan to help rebalance your gut and in turn help with your health concerns. If warranted, other testings and labs may be ordered. In this program, we are not just treating the symptom, we investigate what may be driving it and create a personalized restoration plan.

Visits and Fee - Nurse practitioner Alison Zolcak, leads the Gut Microbiome Restoration program. Appointments for this program will be with nurse practitioner Alison. Gut exploration and restoration is not a one time visit due to its complexities, which is why this specific service was designed. There will be 3 visits for this program. The first visit is in office or virtual and consists of a comprehensive health history review and thorough discussion of current concerns and goals. This visit is 60 minutes of provider time. The GI MAP test is ordered after this visit. Once results are in, they are tediously reviewed and interpreted. A second visit will then take place, which can be in office or virtual. This visit is 60 minutes of provider time. At this visit your unique results will be reviewed and explained. An individualized plan of care, which may include supplements, possible prescription medication (only if necessary), and nutrition plan will be created specifically for you and discussed in detail at the visit. After you have implemented your specific plan, you will have a 3rd and final visit which can be in office or virtual. In addition to checking in on your progress, your provider will also discuss certain topics that can disrupt/improve gut health such as sleep, mood, and environment. This session typically occurs after a few weeks of plan implementation. This visit is 60 minutes of provider time. Fee for the Gut Microbiome Restoration Program is $1000.

Cupping, Acupuncture, Osteopathic Manipulation

Wet fire cupping and osteopathic manipulative treatments are typically used for musculoskeletal concerns and pain. Additionally, acupuncture can be used to help with sinusitis, migraines, and many other ailments. Those interested in this treatment do not need to be functional medicine patients in the practice. Fee is $150.

Nutrition Visits

Are you looking for help in optimizing your health, to feel better overall or need a better understanding of how food can affect your health and feelings of well being? Nutrition visits may be beneficial for you. We start by asking questions and making recommendations that are personal to you. This isn’t a place of prescribing what you have to eat or what you must avoid but rather a way of working with you to find what helps you feel your best and fits your preferences and lifestyle. From what to buy at the grocery store, what to order at your favorite restaurant, to what to cook at home, the goal of these visits is that you feel empowered to make choices that work best for you. This includes two sessions, in the office, video visits or phone calls, whatever fits best into your schedule. Fee is $350.
